For Staff of Faculty of Science, Mahidol University
The Faculty of Science will be applying for a specific insurance plan with SMK Insurance Co., Ltd. (via Mahidol University) as per No. 78.013 / 00075 for the staff of the faculty.
To apply for the insurance, please submit a copy of your ID (Passport or National ID Card) to the Administrative and Clerical Division, room K131, K Building, Phayathai Campus. The copy must be written with "To be used in applying the COVID-19 Jer Jai Job plan with the SMK Insurance Company" and signed.
The document must be delivered to the Administrative and Clerical Division within 25 January 2021 and can be submitted via the following options :
1. Self-delivered to Khun Khamron, Room K131, K Building
