MUSC-IR/1 (Requesting service form)

To request documents (e.g. Recommendation letter for applying VISA, Extension of temporary stay in the Thailand letter) from MUSC, please complete this form and submit it with relevant documents to MUSC-IC

Request form for Recommendation Letter

MUSC students who request a recommendation letter, please fill this form (with relevant documents and MUSC-IR/1) and submit to MUSC-IC.

MUSC-IR/2 (Internship Evaluation Form)

This evaluation is designed to provide feedback on performance and related issues to assist the student. This form is to be completed by the student’s supervisor when the student has completed their required amount of hours at the end of the internship period.

Memorandum of Agreement for Exchanging, Invitation, or Permission to Work

The Memorandum of Agreement (MoA) is established between Student exchange, Postdoctoral Fellows, Short-term internships , or visiting professors and MUSC. They must sign the MoA after they arrive Thailand.

VISA Extension for International Students

As every exchange student and international undergraduate stay in Thailand longer than 90 days, so they will need to apply for the visa extension to continue their stay until the end of their study/exchange period. MUSC-IC will issue the letter to request for their extension of stay.
